CST Care Service

Introduction

The CST-CARE service is offered as an enhancement to the Community Stroke Team (CST). 

 

What is CST-Care

The CST-CARE service provides stroke specialist care in the community. This is a six-week goal based rehabilitation focused care service. We aim to promote independence, working closely with the Community Stroke Team (CST).This is currently a pilot service; we hope this will be adopted to a long-term service in the future.

 

The team

The CST-CARE team consists of:

  • Care Manager

  • Rehabilitation Practitioners

  • Rehabilitation Support Workers

 

Team aims

  • To promote independence in activities of daily living that is based on your goals.

  • Work with you to help you achieve your best recovery and support your ability to live independently at home.
  • To reduce the time you need to stay in hospital, and your reliance on long-term care.

  • Improve your quality of life and mood after a stroke, reducing any psychological impacts of stroke.

  • Work in partnership with you to support and enable you to develop the skills and resources required to self-manage your ongoing rehabilitation, so you can live meaningful, fulfilling and enjoyable life beyond your stroke.

 

The process

  1. Referral to CST-CARE takes place while you are in hospital.

  2. CST-CARE will commence on discharge from hospital.

  3. CST-CARE will do an assessment at home to identify your individual needs and agree goals.  Family and carers can be involved in the process.

  4. We will review your care needs on an ongoing basis throughout your time with CST-CARE.

Other information

Northampton General Hospital operates a smoke-free policy. This means that smoking is not allowed anywhere on the Trust site, this includes all buildings, grounds and car parks.

Car parking at Northampton General Hospital is extremely limited and it is essential to arrive early, allowing ample time for parking. Alternatively, you may find it more convenient to be dropped off and collected.
