Northamptonshire Community Stroke Team

Introduction

Our aim

To provide a stroke rehabilitation service which has quality and equity at its heart and is driven by highly skilled clinicians who feel valued and empowered to make a positive change in the quality of life of our Northants stroke survivors. 

 

 

The team

The team consists of: 

  • Occupational therapists

  • Speach and language therapists

  • Physiotherapists

  • Nurses

  • Rehabilitation assistants

If you need a doctor, please contact your own GP.

 

 

After discharge

After your discharge from hospital, members of the team will visit you in your own home and work with you to develop a rehabilitation programme. The team will discuss your goals with you. We will work with you and your family (or any support people you wish to include) to help you towards achieving your goals. 

You will need to work hard at your rehabilitation programme as this is how you will improve. 

 

The process

 

  • Referral made to the team whilst you are in hospital.

  • Agreement to be seen by the Community Stroke Team.

  • You will be seen in your own home within 48 hours of discharge by a keyworker (a named team member).

  • The keyworker is your main point of contact.

  • Your goals will be planned and agreed. 

  • The rehabilitation process will commence.

  • Your notes will be held in your home and will be completed by the team.

  • You will be given information about other services.

  • You will be discharged when you have achieved the agreed goals. 

We will work with you to improve how you manage your everyday activities in your own environment.

Other information

Northampton General Hospital operates a smoke-free policy. This means that smoking is not allowed anywhere on the Trust site, this includes all buildings, grounds and car parks. Leaflets, information, advice and support on giving up smoking and on nicotine replacement therapy are available from the local Stop Smoking helpline on 0845 6013116, the free national helpline on 0300 123 1044, email: smokefree@nhft.nhs.uk and pharmacies.

 

Car parking at Northampton General Hospital is extremely limited and it is essential to arrive early, allowing ample time for parking. You may find it more convenient to be dropped off and collected.
