Patient & Family Support Service

Introduction

 

This page outlines the Community Stroke Team (CST) patient and family support service at Northampton General Hospital (NGH) and Isebrook Hospital. 

 

Our aims

 

Supporting you and your family on your stroke journey is central to our work as a team. Peer support describes how stroke survivors can help each other to achieve their best outcome and feel supported through sharing lived experiences. Providing holistic support to the family as a whole via advice, referral, group work and provision of information helps the family to feel fully equipped to support the stroke survivors whilst also taking care of themselves. 

 

The team

 

The team consists of:

  • Stroke Ward Volunteers

  • Community Stroke Team Peer Support Worker

  • Northamptonshire Carers Support Worker

 

 

What we do

 

  • Our Peer Support Worker will be your main keyworker to provide advice on information, leaflets, access to equipment and support based on their experience of stroke.

  • Our team of ward volunteers will be on hand daily as a friendly face to provide support through their personal experiences.

  • Our Peer Support Worker and volunteers will help you access our online NGH Stroke Hub so you can access online peer and family support, information about services and ways to learn more about stroke.

  • CST work closely with Northamptonshire Carers Support Workers who will help you to identify the needs of anyone who cares for you and your family.

  • We offer online and face to face groups for all stroke patients and families. You can join a peer support group, some examples are: exercise group, fatigue group, upper limb and lower limb groups.

 

How do you access support?

 

  • You will be offered a referral to these services during your rehabilitation after your stroke.

  • You can ask to have access or to be referred to these services at any time.

  • Information will be available from stroke wards and their volunteers, the internet, NGH Stroke Hub, and the Community Stroke Team patient note pack.

Other information

Northampton General Hospital operates a smoke-free policy. This means that smoking is not allowed anywhere on the Trust site, this includes all buildings, grounds and car parks.

Leaflets, information, advice and support on giving up smoking and on nicotine replacement therapy are available from the Stop Smoking helpline on 0845 6013116, the free national helpline on 0300 123 1044, e-mail: smokefree@nhft.nhs.uk and pharmacies.

Car parking at Northampton General Hospital is extremely limited and it is essential to arrive early, allowing ample time for parking. Alternatively, you may find it more convenient to be dropped off and collected.
