How do referrals come in?

How do referrals come into the Community team?

Referrals can come into the Community team from a number of different places, some of which are:

  • GP Surgeries
  • Nurseries
  • Schools
  • Acute Paediatrician
  • 0-19 pathway

An accepted referral will have all of the following documentation included, this is neccessary in order for a comprehensive assessment to be held.

  • Parent pack
  • School pack
  • Any additional reports specific to your child such as 
      • Speech & Language reports
      • Occupational Therapy reports
      • Educational Psychology reports 

Once a child’s referral has been accepted, they will be placed on a waiting list for an Community Paediatrician appointment, after which, if your child has not been discharged as a result of the appointment, then they will be placed on a secondary waiting list under one of the below pathways. Which pathway will be dependant on their age and assessments needs.
